# Missed a GLP-1 dose? What each label says, and how to keep it from happening again

> The missed-dose rules from the Wegovy, Ozempic, Mounjaro, Zepbound and Saxenda labels, side by side, plus how Peppy computes missed and overdue and nudges you.

Each GLP-1 label has its own missed-dose rule. Ozempic: take it within 5 days, otherwise skip. Mounjaro and Zepbound: within 4 days (96 hours), otherwise skip, and never two doses within 3 days. Wegovy: take it if the next dose is more than 48 hours away, otherwise skip. Saxenda: just take the next daily dose; if more than 3 days have passed, the label says to restart at 0.6 mg. Follow your prescriber.

## The missed GLP-1 dose rules, by label

A missed GLP-1 dose is the most common panic moment for anyone on a weekly shot, and the answer depends on which medication you take. The rules below are quoted from the current US prescribing information. They are the manufacturer's instructions, not ours, and your prescriber can override them for your situation.

| Medication | If you miss a dose | Minimum gap between doses |
| --- | --- | --- |
| Ozempic (semaglutide) | Take it "as soon as possible within 5 days after the missed dose." After 5 days, skip it and take the next dose on the usual day. | At least 2 days (48 hours) |
| Wegovy (semaglutide) | If the next scheduled dose is more than 2 days (48 hours) away, take it as soon as possible. If it is less than 2 days away, do not take it. After two or more consecutive missed doses, resume as scheduled or reinitiate the escalation schedule, as your prescriber decides. | At least 2 days (48 hours) |
| Mounjaro (tirzepatide) | Take it "as soon as possible within 4 days (96 hours) after the missed dose." After 4 days, skip it and take the next dose on the usual day. | At least 3 days (72 hours) |
| Zepbound (tirzepatide) | Same as Mounjaro: within 4 days (96 hours), otherwise skip. | At least 3 days (72 hours) |
| Saxenda (liraglutide, daily) | "Resume the once-daily regimen as prescribed with the next scheduled dose." Do not take an extra dose. If more than 3 days have passed, the label says to reinitiate at 0.6 mg daily and follow the escalation schedule. | Once daily |

Two things are the same on every label: never double up, and when in doubt, call your prescriber. Older Wegovy labels used the same "within 5 days" wording as Ozempic; the current one is written around the 48-hour gap instead, so if you have seen both, that is why.

Compounded [Compounded Semaglutide](https://peppytracker.com/compounds/semaglutide-compounded/) and [Compounded Tirzepatide](https://peppytracker.com/compounds/tirzepatide-compounded/) have no label. Most prescribers hand people the branded rule for the same molecule, but ask yours rather than assuming.

## Why the windows are so long

Weekly GLP-1s are weekly because they leave the body slowly. The Ozempic label puts semaglutide's elimination half-life at about a week; Mounjaro's label puts tirzepatide at about 5 days. A day or two late, most of the previous dose is still circulating, which is why the labels allow a late shot rather than a skipped one. The flip side is that stacking two doses close together adds up, hence the 48-hour and 72-hour minimum gaps. ## "Did I miss it, or did I forget to log it?"

Half the missed-dose worry is not about the medicine at all. It is about memory. Did Thursday's shot happen? A sore spot on the thigh says yes, an empty calendar says no. A tracker only helps if it makes that question unambiguous.

Peppy handles this by never storing a "missed" dose. Your protocol has a schedule (every Thursday, say) and a log of what you actually did: shots you logged as **taken** and shots you logged as **skipped**. Everything else is computed on the fly from those two things:

- **Upcoming:** a scheduled dose in the future.
- **Overdue:** the scheduled time has passed today and there is no log yet. The Today tab's countdown flips to "OVERDUE".
- **Missed:** a scheduled day has gone by with no taken or skipped log.

Because status is derived rather than saved, fixing the record fixes the status. Add the shot you forgot to log and the missed marker vanishes. Log it as skipped and it shows as a deliberate skip rather than a gap. The Calendar can never show a duplicate dose or a phantom missed one.

## How to do this in Peppy

Reminders and nudges are Pro. Computed missed status, the Calendar and adding a past dose are free.

1. Set the protocol's schedule in **Protocols**: pick the compound, the weekday and the time. The **Today** tab now shows a countdown ("Due in 3h 20m") and flips to OVERDUE when the time passes without a log.
2. Turn on **Settings ▸ Reminders** (Pro). Choose the dose reminder time, then a **missed-dose nudge** at 30 minutes, 1 hour or 2 hours after the dose was due. Both notifications carry a **Log it** action; tap it and the shot is recorded at the current time without opening the app.
3. Took the shot but forgot to log it? Open **Calendar**, tap the day, tap **Add a past dose** and set the real time and site. The missed dot turns into a taken dot and your adherence percentage in **Progress** (Pro) updates.
4. Decided with your prescriber to skip? Open the dose from Today or Calendar and tap **Skip**. Skipped doses show with their own marker so you can tell "chose not to" from "forgot".
5. Add the **Next Shot** widget (Pro) to your Lock Screen. It shows the countdown and has a one-tap Log button, which is the cheapest insurance against forgetting a weekly shot.

Peppy records what happened and reminds you what is next. Whether to take a late dose, and when, is a decision for you and your prescriber, using the label rules above as the starting point.

## Frequently asked

### What happens if I miss a GLP-1 dose by one day?

For the weekly pens, one day late is inside every label's catch-up window (5 days for Ozempic, 4 days for Mounjaro and Zepbound, and more than 48 hours before the next dose for Wegovy), so the labels say to take it as soon as you remember and then continue on your usual day. Confirm with your prescriber if you are unsure.

### Can I take two doses of Ozempic or Wegovy to catch up?

No. Every GLP-1 label says not to double up. Mounjaro and Zepbound state doses must be at least 3 days (72 hours) apart; Ozempic and Wegovy say at least 2 days (48 hours). If you have missed more than one weekly dose, the Wegovy label says to talk to your prescriber about resuming or restarting the escalation schedule.

### How do I know if I actually missed a dose or just logged it late?

That is what a tracker is for. Peppy computes missed and overdue from your schedule and your log, so if you took the shot but forgot to log it, you add it to the Calendar with the real time and the missed marker disappears. Nothing is stored as missed; it is always recalculated.

### Does a missed dose reset my titration?

Not automatically. The Wegovy label says that after two or more consecutive missed doses your prescriber may resume as scheduled or restart the escalation. The Saxenda label says to reinitiate at 0.6 mg after a gap of more than 3 days. Any change to your dose step is your prescriber's call; Peppy only records what you tell it.

### Will an app remind me if I forget my shot?

Peppy's Reminders (Pro) send the dose reminder at your chosen time, then a missed-dose nudge 30 minutes, 1 hour or 2 hours later if you have not logged it. The notification has a Log it action so you can record the shot without opening the app.
